Financial and Insurance
We accept ASSIGNMENT of your insurance benefits allowing
your insurance provider to pay directly to our office.
PATIENT COPAYS are due at the time of service.
We estimate your copays when we present your treatment plan.
We accept several forms of payment:
Personal Checks, Cash, Credit Cards, and Debit Cards
Credit Cards accepted are Discover, Master Card, and Visa
PPO Insurance Plans
We are PARTICIPATING PROVIDERS of all PPO Insurance Plans,
which allows you to get the full discount you deserve.
We will be on your PREFERRED PROVIDER LIST.
Most PPO Plans pay 100% for Preventive, with no deductable,
80% for Fillings, Root Canals, and Extractions, and
50% for Crowns, Bridges, Partials, and Dentures, after deductable.
HMO Insurance Plans
We are Contracted with Aetna, Cigna, Assurant, and Dental Source.
You must be on our MONTHLY ROSTER before we can schedule your appointment.
An exception is for EMERGENCY TREATMENT, where you can call your HMO plan and have them fax over your information.
